Introduction

We will visit your premises to assess the food hygiene rating. However, businesses that have been previously assessed as low-risk, and are now due a visit, may receive a self assessment questionnaire instead.

You should complete this form if you have received a letter or email from us. We may also carry out a visit.

You will need to provide the following information:

• details about your business
• if you have made any changes since the last visit
• your supply chain
• staff training
• what controls are in place

You will not be able to save your form. You should ensure you have enough time to be able to complete the form.
